Enter the date and total time which the service was performed.

Enter the name of the place /organization where you served where it says, “Service Org”.

Select a category in which the service fits best. You must select one of the following:

a. Service to the Church
b. Service to the School
c. Service to the Community

Describe the work that you did where it says, “Tell Your Story”.

Answer the prompt, “What is an important lesson that you have learned from this service-learning experience?”

Enter the adult supervisor’s name and email address so they can verify the work you did.

Click on “Submit Hours” and you are all set!
