Medical

The nurse is available daily to provide first aid to students. Students with special medical problems should keep the nurse informed of them.

Medical Forms

Student Emergency Medical Authorization forms must be returned to the school on the first day of school. State law requires that they be kept on file each year. Students who have not returned the form may not be allowed to attend classes.

Medications (Prescription & Non Prescription)

The School Healthcare Provider keeps all prescription medications (with the exception of inhalers and EpiPens) in the medical office. A “general medication form” signed by both the physician and a parent/guardian must be turned in before these may be dispensed. All medication forms are available on our school website and should be turned in at the start of each school year.

The school refuses to permit any child to take any medication during school hours or supervised activities unless and until said forms are furnished. Cardinal Stritch will only dispense non-prescription medicines (Acetaminophen or Ibuprofen…i.e. Tylenol, Advil, etc.) to any student (K – 8 – No preschooler) with written permission granted on the Emergency Medical Authorization Form.

Acetaminophen or Ibuprofen may be provided for pain related to common complaints such as headache, menstrual cramps, or minor joint or muscle pain as long as the emergency medical form states the student may be given it during school hours.

Immunizations and Healthcare Requirements

Preschool and Kindergarten: All preschool and kindergarten students are required to have a current physical and immunization record on file by the 15th day school. Kindergarten students are also required to have a dental exam.

Incoming 7th graders: Incoming 7th graders must have a Tdap booster and meningitis vaccines by the 15th day of school.

Incoming Seniors: Must have two meningitis vaccines prior to the 15th day of school.

These requirements are mandated by the Ohio Department of Health. If your student does not meet these requirements, they may be excluded from school. Remember doctor’s offices get very busy through the month of August.
